Oversight Reporting Form

This form is meant for those who wish to report oversight concerns to CIRA leadership, which can cover anything violating our club’s Code of Conduct, or to raise other concerns one might have. It also covers concerns throughout all of CIRA, no matter which branch of CIRA is concerned. The answers to this form will only be available to the President, Vice President, and Director of Oversight of CIRA.
